Continuums of Care

Continuums of Care (CoC) is a regional strategic planning body to plan and implement housing and services to reduce the incidence of homelessness by assisting individuals, youth and families experiencing homelessness to access services and stable housing. The CoC includes prevention, outreach and assessment, emergency shelter, transitional housing, and permanent supportive housing or other permanent housing. Annually the CoC applies to the U.S. Department of Housing and Urban Development (HUD) Continuum of Care Program for funding to address homelessness. Minnesota Continuums access $38 million annually through this process.
